In my opinion, the use of this picture in the article threatening the President counts as fair use. It is not unlike the situation with the Virgin Killer image. Theoretically, we don't need the picture of that album cover; we could just use text to describe it. And indeed, text is used to describe it: "The original cover art for the album depicted a naked ten-year-old girl named Jaqueline, with a shattered glass crack effect obscuring her genitals." But, we still include the pic anyway. Whether the subject can "be adequately conveyed by text without using the non-free content at all" seems to really be just a matter of opinion; I'm not sure what objective standard we can use to settle this.
