I have obtained the following release from the creator of this image and have forwarded it to permissions-commons@wikimedia.org. Please update the license terms for this image as needed. I cannot find instructions on how to change the terms of an already-uploaded image.

I am aware that this agreement is not limited to Wikipedia or related sites.
