During the semi-finals Huw Richards was elbowed in the head by Gary Whetton, who he then punched before being knocked out by Buck Shelford, after he regained consciousness, Fitzgerald issued Richards the first red card of the tournament.[2] Fitzgerald, went on to referee the World Cup Final.[1]

He went on to referee 26 tests, including the England-Scotland Rugby World Cup Semi-final in 1991, just 6 weeks before his death.[3]
