What is the relationship between Common Toxicity Criteria (CTC) and Common Terminology Criteria for Adverse Events (CTCAE)?

CTCAE version 4.0 was published 28-May-2009, and version 4.03 was published on 14-Jun-2010 reference http://evs.nci.nih.gov/ftp1/CTCAE/About.html Please update this entry, as it is 13 months out of date.
