RfC: Split and merge[edit]

Diabetes being only partially related to Cardiovascular diseases, vice versa, I'm proposing to reorganize the content in this area as follows:

rename this page to Diabetes in Australia
split apart the section about Cardiovascular disease, merging it into Heart disease in Australia
rename the latter to Cardiovascular disease in Australia
