Using Google translate from the Spanish, I see this:

"Regarding the ownership of the contents, the user can occupy and make use of them, as long as TheClinic.cl, the author of the publication, is cited and the respective link to the referred content is included."

This is not indicating a public domain image or Creative Commons Attribution-Share Alike 4.0 International. Wikipedia Commons will need to delete this image unless you can get it fully release via a process such as this:
