You really should have used the appeal form above. You need to make it clear that you won't write about yourself anymore and that you will comply with WP:VERIFY and WP:BLP (showing that you've read them). That's how to get unblocked. Dougweller (talk) 13:48, 19 April 2013 (UTC)
