Pneumonia

I have had a go at the pneumonia investigation section (see entry in talk page) as this was very much written from a medical textbook/hospital approach, whereas most cases are treated in the community. Hence most cases (in UK) get no investigations, sputum culture often done, Xray less frequently and the rest rarely. The original was shorter, but not what a patient (ie wikipedia viewer) might typical expect. Please do have a look at my edit and change if structure/english needs improving, I really wont mind :-) David Ruben 19:58, 17 August 2005 (UTC)

Medical record

There is a suggestion to merge Medical record, that you have previously contributed to, with Medical records. There is a big difference between what makes up an individual clinical record (history, examination, investigations, correspondence) and all the administrative/legal aspects (keeping, disclosing vs confidentiality, auditing of records). Merging may may create an article that is too long, but separately the names are too cofusingly similar. There is also some comments about moving the details of examination to Physical examination. Siderosis?

Hi, have run out of time to work quite so intensely on human iron metabolism for a while--but in a quick search didn't find siderosis in Up-to-Date list of iron overload syndromes... I think that the syndrome formerly known as "Bantu siderosis" is now "African iron overload" in these parts (US) anyway for reasons having to do with the South African politics of the term "Bantu" --dx is apparently a combination of genetic and environmental factors, I'll try to broaden that last paragraph enough to include it. Ultimately we also need to update the iron overload page to expand more of the non-hemochromatosis causes. Brief web search shows siderosis as defined as the result of inhalation of iron particles. The question is, should this article have all the related diseases, or should it try to link out from a list? My idea about the closing was just to have a brief bit about the clinical significance of the basic science discoveries... but perhaps the iron diseases should be a bigger part? Let me know what you think. Also, I put a request for review here--is this being used at this point, or is there a better place to get medical-ish folks to take a look? Also, inspired by your note I revised the hemochromatosis bit on African iron overload and created an African iron overload stub, so it was a good example for generating some other stuff, at the very least! Anyway, along the lines of the implicit nature of your question re: broadening the clinical portion--It looks like the definition of siderosis from the example of transfusional siderosis cited in the article above is really, acquired iron overload--which leads me to think maybe it would be better to do a genetic overload/acquired overload part of the metabolism article.

The most effective place to get peoples attention would be the doctors' mess I think. Also to get more general feedback from non-medical people, you could put it on peer review (and who knows, even turn it into a featured article). About the article: I don't think the article needs to cover all related diseases, but mentioning more than just one and some general info about diseases related to iron metabolism would be nice. Human chromosome pages

Hey, I've noticed you've started adding some human chromosome stubs and it just struck me that you could add a nice template at the bottom of each stub/article (if you fancied it) so that people could navigate amongst the chromosomes easily. A bit like a football 'succession' box, (see Bobby Robson for a good example!). I know zero, zip, nada, nothing about human chromosomes but it seemed like a good idea and I'd certainly be interested in seeing it! Besides that, looking forward to learning more! Naming system

I see you have done a lot of work on molecular and cell biology related articles. We have to find a standart way to reffer to the gene and the protein names. PPOX Article

Yeah I'm the one who reported it as copyvio. I know it is a government source and technically we should be able to copy-paste those here. But the terms of use at http://ghr.nlm.nih.gov/ghr/page/TermsAndCs are incompatible with wikipedia. QBorg 21:52, 2 January 2006 (UTC)

I think the problem is that ghr.nlm.nih.gov compile informations from different sources and the copyright to those is not owned by them as they seem to say on [2] Both pages seems to be contradictory thought... QBorg 22:25, 2 January 2006 (UTC)

Hand Surgery

Hello! The content of your article is confusing - does it relate to surgeries that are performed on the hand or surgeries that are carried out by hand as opposed to endoscopic, for example? p53

Hi, I noticed that you added a page called "TP53" at the end of December last year, covering the gene/protein p53. There is already an excellent entry for "p53" which is why is changed your page into a REDIRECT to the page "p53" which seems to cover all and more that you write about. Hope you don't mind. I know about the p53 article. I created TP53 as an article for the gene rather than the protein, but indeed most contents were the same although with a little different emphasis. I don't really know if they should have seperate articles. --WS 21:02, 16 January 2006 (UTC)

PMID citation bookmarklet

I gave the bookmarklet a shot in Firefox 1.5 -- it works very nicely. Regarding the problem you're experiencing with it (the fact that it treats the last number in the URL as the PMID when it should really be using whatever is after "list_uids="), I'm no JS whiz, but here's what I'd use to fetch the pmid:

I was recently asked if there was information or a guidebook anywhere that would help a user understand how to locate a reference article and insert the reference to it. I was going to direct the user to the discussion on your bookmarklet for the PubMed tool but Clinical Medicine's talk page was recently archived. Given that this is an ongoing tool to help with medical articles, I have copied and reorganised the details into the Project's front page under the section about references, here. I think any improvements to the explanation or the javascript should be as edits, rather than as a sequential series of entries seen in a talk page's discussion. Picture moving

I have just moved Cephalexin to its International Nonproprietary Name of Cefalexin (as per naming of other cephalosporins on wikipedia) and also cut out the verbosity of USA direct marketing literature warnings. However the picture used of the molecule is Image:Cephalexin.png. Is there anyway to move or rename pictures in wikipedia (there is no 'move' box in my browser's left-frame as there is for articles) ? David Ruben ^Talk 12:10, 25 January 2006 (UTC)

VWN en WCN

Al enige tijd is er een Nederlandstalig chapter in oprichting, te vinden op http://nl.wikimedia.org . Dit wordt de Vereniging Wikimedia Nederland (VWN). Je kunt je interesse om lid te worden van deze vereniging hier aangeven.

Deze vereniging gaat eind augustus/begin september een Wikimedia Conferentie in Nederland (WCN) houden, volgend op Wikimania in Boston, gedeeltelijk erop inspelend middels een aantal discussiegroepen. Om iets dergelijks te organiseren is imput erg gewenst. Dus als je wilt meehelpen, of als je interesse hebt om bij een dergelijk evenement aanwezig te zijn, geef dat dan aan op nl.wikimedia. Venipuncture article

I noticed you thought there should be information on other blood collection systems than Vacutainer in the venipuncture article. I've been working on both the phlebotomy (modern) and venipuncture articles, and I'd appreciate it if you could supply your own knowledge of other systems. I live in the U.S., and I have only used the Vacutainer or Vacutainer compatible vacuum tubes and syringes to draw blood. BrianGCrawfordMA 17:28, 26 February 2006 (UTC)

Ik heb me scheelgezocht naar een goede bron over de i-kanalen waarop ivabradine zou moeten aangrijpen. Welke bron heb jij gebruikt voor deze correctie? Ik ben erg benieuwd. OMIM gaf niet thuis.

In het geneesmiddelenbulletin dat altijd bij medisch contact zit, stond een stukje over de registratie van ivabradine, daar heb ik het uitgehaald, veel meer dan dit stond er niet vermeld over de ionen kanalen. Coschappen bevallen prima, ben alweer bijna klaar met psychiatrie, binnenkort dermatologie en daarna interne. Het geneesmiddelenbulletin! Deksels, het bestaat dus toch nog! In het VK heeft de overheid net de subsidie for het "Drugs & Therapeutics Bulletin" stopgezet, de lokale tegenvoeter van het Gebu. Ikzelf ontvang het niet.

Psychiatrie was vast interessant. Ik bracht 6 weken door in een gesloten afdeling, waar mijn favoriete patiënt (een schizofreen met veel negatieve symptomen en nul ziekteinzicht) bilaterale otitis externa ontwikkelde omdat hij zijn hoofdtelefoon niet wilde afzetten. Hij kreeg zulke interessante berichten via zijn muziek, direct aan hem gericht!

Waar doe je interne? Het is verbazend hoe moeilijk het is om dermatologie goed te leren. Wij hebben met de regelmaat van de klok patiënten met merkwaardige erupties, maar niemand weet precies wat je ermee aanmoet. Een huisarts vertrouwde me ooit toe: "Dermatologie bestaat uit twee ziektebeelden: de uitslag waar je steroïden opsmeert, en de uitslag waar je geen steroïden opsmeert." hi there. in case you missed it, i thought you might like to know AIDS is appearing on the main page as a featured article on June 15th (this month). if you could, it'd prolly help to have someone keeping an eye out for vandals specifically on that date, since as of right now there is at least one vandalizing a day since it's FA status. now would also be a good time to copyedit it before the big slam on its main page day. :) JoeSmack ^Talk 16:32, 4 June 2006 (UTC)

I looked at a "Bariatric Surgery" search, which reflexes to "Bariatrics". However, the page is almost entirely about bariatric surgery! Bariatrics is a vast area of non-surgical knowledge, and should have its own page, separate from Bariatric surgery. I'm not sure how to do this, and I am not cognizant of bariatric medicine, sufficient to write a page about it, but I could create a basic stub page, which would allow it to be expanded by an appropriate contributor. I think there's no doubt the sections should be split into separate topics, and I noticed too that they were recently merged, which was a mistake. However, in order to split them, it will require a rewrite of a whole new section on bariatric medicine, which gets short shrift on the current page. As a bariatric surgeon, I'm less than a strong believer in the alternative field, and probably not knowledgeable enough to write more than a basic starter page. Gastric Bypass Globalization

I appreciate that Wikipedia is a global resource (one does tend to forget). However, it will be difficult to globalize the section on Centers of Excellence, because there is no international counterpart, yet. Bariatric surgery originated in the USA, and the ASBS was the founding professional organization. Overseas surgeons initially became members of ASBS, and then the International Federation for Surgery of Obesity was formed, with national member societies in many countries. IFSO was initially an offshoot of ASBS, and as an ASBS member, I am a member of IFSO. Many foreign surgeons continue to maintain a membership in ASBS, in addition to their own national societies and IFSO.

I can put in a paragraph about IFSO, and a link to their website [3], which is operated as a subpage of a privately operated info website out of Austria, by Dr. Karl Miller [4]. It does not even have a listing of member surgeons, except national society officers. It has no referral service - surgeons must list with the parent site, for a fee. I'll inquire further next week, at the ASBS annual meeting.

Center of Excellence is a new concept even in the USA, and there is no counterpart, to my knowledge, in other countries. However, the Medicare adminstration has already endorsed the concept, by limiting re-imbursement only to designnated centers. Other insurance is sure to follow, and the concept is certainly valid, but the justification is complex and probably beyond the scope of WP. Topnife 15:50, 22 June 2006 (UTC)

WS, I noticed you switched treatment and prognosis, which is a subject that has come up in other conversations about the TS article. I think (? can't recall for sure) that the medical article guidelines indicate that the order can be changed when indicated. My concern about the order of the sections is that, unless one understands the optimistic prognosis for most persons with TS, the context is not clear for most of the treatment decisions. In this case, I think the optimistic prognosis has to be established first, in order to understand the typical "wait and see" approach to treatment. Let me know if you disagree: I'm hard at work trying to tie up the loose end in the article (still doing a major rewrite of Prevention/Screeing so that I can work in ADHD, OCD comorbid issues, and wish that Encephalon would show up with his PANDAS rework), so let me know if you see other weaknesses I need to address. The new and third article on Legionnaires' disease

I am writing to you because, in the past, you have contributed to the article Legionellosis and therefore might be interested in the following:

Wikipedia has had two existing articles for quite a while named Legionella and Legionellosis. About two months ago, User:Noigel2000 started making changes to both these articles with no prior discussion on the talk pages. When his changes got edited, he became quite perturbed. After quite a bit of discourse, (during which his need to use proper Wiki syntax and guidelines was pointed out to him), he became even more perturbed. He made a number of derogatory remarks about other editors and about Wikipedia, and kept insisting that his Internet website (devoted to Legionnaires' disease) was the best source of information and better than anything in Wikipedia.

Finally, Noigel got tired of having his edits to the two existing articles corrected and of being admonished for some of his remarks. So he wrote a third, completely new article entitled Legionnaires' disease. It is composed entirely of material taken directly from the many pages on his Internet web site. He made absolutely no attempt to "Wikify" the article. There are no references, no external links (with one exception, a link to his web site), no embedded Wiki links, poor organization, no use of Wiki section headings, etc. In my opinion, he is using Wikipedia as a billboard for his web site (and I admit that I am biased, because I do not like his attitude).

FAST

Great article! When I was a medical resident, I was having difficulty landmarking a thoracentesis, so I grabbed the FAST machine (no trauma going on) and tried to locate it. Found the pleural efffusion, but the director of the ER found me (as a trauma came in the interim), and we had to have a chat about "appropriate use of hospital resources"... doh. Immune system

Hi, I just wanted to comment on your edit to Immune system. There have been several discussions on the talk page about this very topic. Because the general compontents are nearly identical, it would be safe to say that this article focuses primarily on the immune system of mammals, but not just humans. Also, there is some mention of plants and eukaryotes in the innate section; putting human in the description only confuses the reader. I have planned on creating a seperate article for plant immunology/defense, but I think that the consensus was that host defense in lower organisms was outside the scope of the article. I would certainly welcome any help you have to offer though, in creating a plant defense (bacterial defense, ect..) article, I have to admit my knowlegde of this are is woefully inadequate. LOTS wrong with it: there have been very long Usenet discussions about it, involving respected TS physicians and professionals. Rather than lay it all out on your talk page, should I detail it all on the article talk page? The diseases DB is more straightforward, since it's six years outdated. It's always been a mystery why the NIH fact sheets are so well maintained and accurate, yet the OMIM entry is not - maybe maintained by an entirely different group of people? I started to outline problems on the talk page, but the fundamental problem is that the OMIM database gives undue weight to old Comings' research, and his research didn't even enjoy the wide acceptance of his peers when his research was new -- he wrote an entire book which details his problems in getting his work accepted and published, and he self-published via his own press, Hope Press. Amount of lymph passing through thoracic duct

Hi, I saw that you added the following to thoracic duct a while ago: "In an adult, the thoracic duct transports up to 4 L of lymph per day." Would you have a reference for that? I'm asking because I find widely differing numbers on the net and I don't have access to anything authoritative right now. Marfan Syndrome Article Graffiti Attacks

I have reviewed the history of changes on the Marfan Syndrome article over the past few weeks and have noticed an occasional graffito is written into the article. The vandals perpetrating these attacks usually do it without logging in, thus leaving no contributor trace. I recommend that whenever any of the responsibly acting contributors log in to do some more editing, first look at the most recent edit. I don't think the access date is relevant for articles, they don't change, they are the same no matter what date you accessed them. Anyway if you want that feature you should ask User:Diberri, it is his website which does the actual formatting. --WS 22:31, 13 April 2007 (UTC)

Het ligt waarschijnlijk iets ingewikkelder en is niet alleen afhankelijk van fucosyltransferase, maar: "~7% of the population are Lewis-negative and have undetectable concentrations of CA 19-9 regardless of the tumor burden" PMID 9895338 (zie ook PMID 3166398 PMID 9458099 en nog een heel aantal andere)

Autism epidemiology

Your recent edits to Autism made several nice changes, but I have some qualms about some of them:

• The following text was removed with the comment being only "removed untrue statement":

Autism's incidence, despite its advantages for assessing risk, is less useful in autism epidemiology, as the disorder starts long before it is diagnosed, and the gap between initiation and diagnosis is influenced by many factors unrelated to risk.

But this cites Newschaffer et al. 2007, a high-quality source. Here is a direct quote from that source:

The most commonly reported measures of autism frequency are point prevalence or period prevalence. Incidence rates, despite their theoretical advantages for studying risk, are of more limited utility in autism epidemiology because not only is autism diagnosis distal to disease initiation but also time between initiation and diagnosis is likely influenced by a wide range of other factors potentially unrelated to risk. Cumulative incidence, however, may be informative for descriptive epidemiologic studies of birth cohorts.

The point is an important one, so if the Autism statement doesn't reflect the source accurately, then let's fix it, not remove it. You can find a more discussion on this issue at Talk:Autism/History as of 2007 July 31 #usefulness of epidemiological statistics.

It is impossible to say in general whether one specific statistical measure is useful or not, it all depends on what purpose you use it for. In the context of that specific article, that deals mostly with causes and risk factors, incidence is less useful than prevalence indeed. • Prevalence estimates for related disorders were removed, using the argument that these statistics belong in Autism spectrum disorders. But these estimates are also useful here, to give readers a feel for how common autism is compared to closely related disorders. There is some dispute about whether Autism should be separate from Asperger's (this is discussed in Autism #Classification) and PDD-NOS is often just autism with a symptom or two missing. So the prevalence estimates for these related disorders are relevant and interesting information here.

These prevalence rates are of course all interesting, but as I said, I think the specific rates of all of them don't belong here. Autism spectrum disorders (which needs some drastic improvements) is the best place to compare rates of the different disorders. --WS 23:43, 9 August 2007 (UTC)

• The terminology "real prevalence" was introduced. I don't see why this change is needed. Epidemiology does not have a standard notion for "real prevalence"; there is just "prevalence".

That is mentioned twice more: some of the observed increase in autism in California was real and real changes in autism's prevalence.. I just wanted to make clear the difference between the estimated prevalence and the actual "real" prevalence, as earlier it is discussed that prevalence rates have increased..

• The word "warning" was replaced by "the first", resulting in the sentence "Parents usually notice the first signs in the first year or two of their child's life." with the comment "parents don't experience 'warning signs'". But the original sentence didn't say parents experience warning signs; it said they notice them, with the implication (which I thought was a clear one) that they notice them in their children. Perhaps the sentence could be reworded for clarity, but its current version is worse than the earlier one, since it repeats "the first" in a confusing way.

I just don't think the concept "warning sign" is appropriate here. Few parents will have a notion of what a warning sign is, they rather notice some things about their child that make him different. A warning sign sounds more like something a doctor would use. But indeed this could be reworded better. --WS 23:54, 9 August 2007 (UTC)
