Development[edit]

On April 2, 2024, ABC renewed the series for a twenty-first season with Meg Marinis returning as showrunner. On July 9, it was announced that the network had ordered 18 episodes for the season.[5]

Casting[edit]

On May 13, 2024, it was reported that Jake Borelli will not be returning as a series regular for the season[6]; Two days later on May 15, it was reported that Midori Francis is also not gonna be returning as a series regular[7]. Borelli had been on the show since Season 14, and was promoted to series regular on Season 16, while Francis joined the main cast on Season 19; both actors are expected to return in a guest capacity to finish with their character's storylines. On June 12, 2024, it was reported that Jason George was repromoted to series regular following the cancellation of Station 19 [3]. On July 9, 2024, it was announced that series' protagonist Ellen Pompeo is set to have an increased presence on the series since Season 18, with her appearing in 7 episodes out of the 18 confirmed for the season, with the option to double-up her appearences to 14 also being discussed[8]. On May 13, it was announced that as part of a cost-cutting move, several of the veteran series regulars would face a reduction of their episodic guarantee (the number of episodic fees per season each series regular is entitled to per their contract), likely resulting in less appearances by said regulars throughout the season.[6]
