Reference for Clifford generators[edit]

It seems that reference [4], given as a reference for the Clifford generators, is a lecture from Prof. Aram Harrow's Course in which he just states that the S, H and CNOT generate the whole Clifford group without giving or directing to any proof. The notes just mention that the proof will appear as an exercise in the next problem set which I could not find online... a classic example of "left as an exercise to the reader".
