Concerns

I have another concern -- the agingwithdignity.org web site FAQ ( at http://www.agingwithdignity.org/faqs.php ) says in answer to the question "May I copy Five Wishes?"

'"If you wish to copy the blank document and then fill it out, no. If you wish to copy your completed, signed and witnessed Five Wishes, yes.
Five Wishes is protected by copyright (see notice on the back cover). It is a violation of U.S. law to make a copy of the blank original. Also, health care providers may ask to see the original, signed and witnessed document, particularly if a question arises about whether the document has been changed or altered from the original. Health care providers often want to make their copy from your original and then place it into your medical file."'
