Statement on copyright: this is a standard assay. I've seen some variation in workding between mRS forms for different institutions, so I've altered the wording slightly to (I think) avoid copyright worries. Note, BTW, that the site identified by Corenbot (http://www.strokecenter.org/trials/scales/rankin.html) includes a clear statement that they are not the copyright holders. unassisted walking
