Cause of crash[edit]

The cause of the crash was due to the time-expired engine fitter - who Forrester had had an argument with - failing to change the engine oil cooler as he was ordered to do. He was going back to the UK the next day and so wasn't going to have to 'carry the can' for anything that happened to Forrester, so he deliberately didn't bother changing it. On the subsequent flight the oil cooler leaked oil, and the engine caught fire.
