The inherent rate of the AV node is actually between 40 and 60; this is considered a junctional escape rhythm. The idioventricular rhythm's inherent rate is the one that's between 20 and 40.

I don't think a ventricular escape beat is always due to "failure of electrical conduction between the SA node and AV node". For example, in 3rd degree AV-block, the conduction between the SA and AV node is normal, but the ventricles aren't stimulated. Agreed. It is due to the failure of the next impulse to reach the ventricles in a timely fashion; this failure could be due to a lack of initiation or to a lack of transmission, but ultimately whatever should be happening at the AV node does not arrive at the ventricles.
