The template can be placed in different locations and, depending on what is required, the parameters can be mixed and matched:

Basic reference format, suitable when referencing multiple entries from the same work. For inline referencing, you can use something like {{sfn|New Cambridge Medieval History|loc=Vol. I, Chapter XXX, p. y}}
References section or inline citation {{New Cambridge Medieval History|volume=|chapter=|page(s)=|first=|last=|authorlink=|url=}} Fouracre, Paul (2005). "Introduction: the history of Europe 500–700". In Fouracre, Paul (ed.). The New Cambridge Medieval History, Volume 1, c.500–c.700. Cambridge: Cambridge University Press. pp. 1–12. ISBN 978-1-13905393-8. Some additional information: chapter title; chapter url to the CUP onoine index; the first and last names of the author(s) of the chapter; and the chapter page numbers. Volume number automatically fills the rest of the reference with the relevant data such as editors, publication year, isbn, etc. For inline referencing, use the author last name(s), e.g. {{sfn|Fouracre|2005|p=5}}

This template is wrapper around {{cite book}}. In its default mode it attributes text to an entry (chapter) from the New Cambridge Medieval History.
