Gliadel Wafer draft article[edit]

I am reviewing the Gliadel Wafer draft article.

I note some significant problems:

  • In the section Comparison with other treatments the statement is made that "Gliadel wafer treatment is superior to systemic chemotherapy by delivering a higher drug concentration to tissue adjacent to the original tumor's location". This is a very strong, unequivocal statement. It would need to be supported by multiple reliable sources that clearly establish a medical consensus on this superiority. Currently there is exactly one cited reference for this statement: Dorner, Lutz; Stephan Ulmer, Axel Rohr, Maximilian Mehdorn, Arya, Nabavi (12). "Space-occupying cyst development in the resection cavity of malignant gliomas following Gliadel". Journal of Clinical Neuroscience 18 (3): 347-351. However, the abstract to that article does not mention any comparison of value in treatment. It deals entirely with cyst formation under treatment and how to deal with the cysts. While it may be that such a comparison is made in the body of the article, only the abstract is available to non-subscribers. In any case, a mention that the authors did not consider significant enough to include in the abstract is probably not enough support for such a strong statement. You will want to revise the statement or provide additional supporting citations. If any citations refer to sections of an article behind a paywall, quoting a relevant sentence or two can be very helpful.
  • In the section Efficacy the statement is made that "While the efficacy of Gliadel has been debated, it has been shown to have a small but statistically significant improvement in survival time." However the cited reference for this statement (Kunwar, Sandeep; Chang, Susan; Westphal, Manfred; Vogelbaum, Michael; Sampson, John; Barnett, Gene; et al. (02/04/2010). "Phase III randomized trial of CED of IL13-PE38QQR vs Gliadel wafers for recurrent glioblastoma". Neurooncology 12 (8): 871-881.) says "Median survival was 36.4 weeks (9.1 months) for CB and 35.3 weeks (8.8 months) for GW (P = .476). For the efficacy evaluable population, the median survival was 45.3 weeks (11.3 months) for CB and 39.8 weeks (10 months) for GW (P = .310)." and "There was no survival difference between CB administered via CED and GW." This does not seem to support the statement in the draft article. Cited references should clearly support the statements for which they are cited.
  • I have removed the detailed step-by-stem instructions in the Surgical Implantation Procedure section. Wikipedia is not a suitable place to instruct surgeons or physicians on how to treat patients, and this level of detail would not be useful to anyone else. In any case Wikipedia is not an instruction manual and How-to's or lists of instructiosn are generally not accepted.

These are significant problems with the draft article. DES (talk) 23:53, 7 August 2012 (UTC)[reply]

I am going to decline the article at this time because the sources do not sufficiently support the statements made. DES (talk) 00:10, 8 August 2012 (UTC)[reply]
