Your abstract should not have a heading. The lack of heading is what makes it pop up above the table of contents box.
This would benefit from some copy editing to catch grammar and clarity issues. You may want to work with a friend or with the writing center to read this back over and find those issues.
Whenever you do need to use a medical term, reference any specific condition, body part, or treatment, link to the wikipedia article for it the first time you use it! In general, more links are better.

"disruption of the biliary tree" in pathophys could use more explanation and detail.
While there's not a lot of research on biloma, given its rarity, there is some. Make sure you summarize that in your research section. If you're having trouble finding it, come see me!
You have a figure here, but it doesn't have any labeling or information. It needs a caption and alt-text. Also, if it's relevant, you may want to move it to the top instead of the bottom.

I really liked how concise and summarized your abstract was! I also liked how you incorporated the history of how the term biloma was coined to relate it to its current medical definition.

I would suggest linking a few more terms throughout your article in case readers do not have any medical knowledge or background whatsoever (example: jaundice, anorexia, fibrosis).

Unless there is no research available for bilomas given their very rare occurrences, I would suggest further looking through sources such as Google scholar to see if there is at least one research article you can refer to to highlight what research has been done so far!

For your pathophysiology section, maybe you can quickly explain how disruption of the bile tree occurs to further cause a biloma and bile leakage into the surrounding tissues. I think this would really paint a picture of exactly how bile leaks into the abdominal cavity.

There was a lot of thorough information on this article and the abstract was well-written and allowed for a summarized understanding of the condition. Some edits I would suggest is adding something on the research directions, if there is no future research plans maybe elaborate on the findings presented in present researchers. I would also review the word choices, some would need linking to other wikipedia articles or just some grammar fix.
