Good job. I would use a page like this. I have several times asked Flickr users if they'd consider relicensing their photos but I just usually say what licence is required and what article I'd like to use it on. Often they tell me they know nothing about using Wikipedia (!) and sometimes they re-licence it non-commercial and then I have to say thanks but … .
